Sign up for a time when Tammy Spencer can call you and go over your Kindergarten Registration paperwork. Ms. Tammy will email you the Enrollment Data Form before your call and will need that and all required documents emailed to her at firstname.lastname@example.org before your phone appointment.
You will need to email Ms. Tammy copies of the following information:
- Certified Birth Certificate (not mother's copy)
- Social Security Card
- TN Immunization Certificate and Proof of Physical
- Custody or Guardianship paperwork - if needed
Ms. Tammy will also need: TWO Proofs of Residency
- MLGW Current (NOT Bartlett Water)
- Mortgage Statement
- Lease or Rental Agreement
- Real Estate Tax Receipt
- Government Assistance Communication
All new Shared Residency applications will need to be approved by Student Services first. Check their website for more information.
